Claim Materials

 

Home Insurance:

  1. Copy of the policy.
  2. Photos showing the accident and  damaged items, in case of burglary, photos showing violence being used to break into the property.
  3. Other documents which issued by authority.
  4. If 3rd party is responsible for the damage, correspondence to hold them liable for the damage.
  5. Invoices for the damaged items.
  6. If the items are repaired, the repair invoice.
  7. Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.
 

Personal Accident Insurance:

Section I  Personal Accident:

  1. Death – the beneficiary shall fill in the application form and submit the following documents to the company at their own cost:
    a) Copy of the policy.
    b) Residential Certificate and Identity Card of the beneficiary.
    c) The Death Certificate issued by the policy or other organisation which is approved by the insurer.
    d) If the insured's death was caused by an accident and the body can not be found, the beneficiary must submit the certificate issued by the court.
    e) Certificate on the cancellation of residential account;
    f) Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident
  2. Disability - the insured shall fill in the application form and submit the following documents to the company:
    a) Copy of the policy.
    b) Residential Certificate and Identity Card of the insured.
    c) The Disability Certificate issued by grade 2 or above hospital or other organisation which is approved by the insurer.
    d) Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.

Section II  Double Compensation:

  1. The insured, or the beneficiary or their agents shall fill in the application form and submit the following documents to the company at their own cost:
    a) Copy of the policy.
    b) Receipt of premium.
    c) Residential Certificate and Identity Card of the insured.
  2. In addition to the above, for different types of compensation, the insured and/or beneficiary has to provide the following documents:
    a) For death compensation,you must submit: The Death Certificate issued by the policy or other organisation which is approved by the insurer (If the insured's death was caused by an accident and the body can not be found, the beneficiary must submit the certificate issued by the court and Certificate on the cancellation of residential account (If death is declared by the court, the residential cancellation is not necessary).
    b) For disability compensation: The Disability Certificate issued by grade 2 or above hospital or other organisation which is approved by the insurer.
  3. Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.

Section 3 Personal Accident Medical Expenses:

The insured, or the beneficiary or their agents shall fill in the application form and submit the following documents to the company at their own cost:
a) Copy of the policy and the extension cover.
b) Insured's identity card copy.
c) Original invoices and breakdown of medical expenses list issued by grade 2 or above hospital or the hospital approved by the insurer.
d) Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.

 

Business Safe Insurance:

Section I  Fire and the Perils

  1. Claim Form (Download).
  2. Certificate issued by the authorities, such as Fire Department’s fire report, Weather Bureau's bad weather report etc.
  3. Value of the damaged property, such as invoices, purchase contracts etc.
  4. Repair plan, quotation, or invoice of the damaged property.
  5. Replacement invoice to the damaged property.
  6. Photos of the scene and damaged property.
  7. Other documents necessary to prove the claim.

Section 2  Loss of Rent

  1. Claim Form (Download).
  2. Certificate issued by the authorities, such as Fire Department's fire report, Weather Bureau's bad weather report etc.
  3. Lease contract (as lessee).
  4. Contract of leasing other premise.
  5. Account records or other documents which the insurer needs to investigate or verify the claim.
  6. Photos of the scene and damaged property.
  7. Other documents necessary to prove the claim.

Section 3  Burglary and Robbery

  1. Claim Form (Download).
  2. Police report.
  3. Value of the damaged property, such as invoices, purchase contracts etc.
  4. Repair plan, quotation, or invoices of the damaged property.
  5. Replacement invoice to the damaged property.
  6. Photos of the scene and damaged property.
  7. Other documents necessary to prove the claim.

Section 4  Money

  1. Claim Form (Download).
  2. Police report.
  3. Account records.
  4. Photos of the scene.
  5. Other documents necessary to prove the claim.

Section 5  Public Liability

  1. Claim Form (Download).
  2. Letters, facsimiles, emails etc with third parties.
  3. Documents relating to the loss or the injury.
  4. Photos of the scene, injury or the damaged property.
  5. Other documents necessary to prove the claim.

 

Property and Business Interruption Insurance:

  1. Copy of the policy.
  2. Claim notification form (Download).
  3. Last year’s financial report.
  4. The product and sales records from the date of loss till the full recovery of production.
  5. The standard turnover and sales record during the indemnity period.
  6. Photos showing the accident and damaged items.
  7. Certificate issued by the authorities, such as police report in case of a burglary, Fire Department's report, etc.
  8. If 3rd party is responsible for the damage, correspondence to hold them liable for the damage.
  9. The invoices for the damaged items.
  10. In case of repair, the repair quotation and/or invoices.
  11. Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.

 

Construction/Erection All Risks Insurance:

  1. Copy of insurance policy.
  2. Claim notification form (Download).
  3. Photos showing the accident site and the damaged items.
  4. Technical analysis of the accident.
  5. Contract of the works.
  6. Progress report or the works.
  7. Repair quotation and/or invoices for the damaged parts.
  8. Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.

 

Liability Insurance:

  1. Copy of insurance policy.
  2. Claim notification form (Download).
  3. Photos showing the accident site and the damaged items or injured person(s).
  4. Certificate issued by the authorities, such as police report in case of a burglary, Fire Department's report, etc.
  5. The claim letter submitted by 3rd party.
  6. Any invoices showing the amount of damaged property.
  7. In case of bodily injury, the medical reports and invoices issued by the hospital.
  8. If the damaged items are repaired, the repair quotation and invoices.
  9. Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.

 

Marine Cargo/Inland Transit Insurance:

  1. Copy of insurance policy.
  2. Claim notification form (Download).
  3. Transportation contract, such as Bill of Lading, Air Waybill, Inland Transit Waybill, etc.
  4. Invoice of the shipment.
  5. Packing List/Weight Memo.
  6. Photos showing the damage.
  7. Any damaged report/Tally report issued by the port authority.
  8. Certificate issued by the authorities, such as police report in case of a burglary, Fire Department's report, etc.
  9. If 3rd party is responsible for the damage, correspondence to hold them liable for the damage.
  10. Repair quotation and/or invoices for the damaged parts.
  11. Other documents which the insurer deems necessary to prove the cause and extent of loss in the accident.
